Advance Medical Directive for Choosing C0vid 19 ICU Protocol
In light of recent events it has become important for some to have a say in how they will be treated should they come down with c0vid 19 and require hospitalization. There are protocols being given by major health organizations such as the CDC and NIH that you may or may not feel are right for you. America's Frontline Doctors and other medical professionals have put out their own suggested protocols and many pay prefer to use those. This document allows you to accept or prohibit the NIH and CDC recommended protocols for C0vid 19 and to write in or accept treatment according to the FLCCC, Frontline Doctors or other protocols instead. It also allows you to set a proxy agent to advocate for you should you become unable to speak for yourself, and allows you to write in additional instructions for your care.
